Rejected RMA's are due to the following:
- Incomplete name: Any form submitted missing a full first and last name will be rejected. Please make sure to include first and last name for shipping purposes.

- Incomplete address: Any form submitted with an invalid address will be rejected.
- P.O. box addressed: We DO NOT ship to P.O. Box addresses, please provide a permanent physical address.
